ABSTRACT

Objective@#To observe the clinical effect and safety of Shanhaidan Granules (SHDG) combined with tadalafil tablets (TT) in the treatment of ED.@*METHODS@#In this open multi-center case-control clinical trial, we enrolled 247 ED patients according to the designed criteria, and treated them orally with SHDG at 10 g per time tid (n = 74), TT at 5 mg per time bid (n = 52), or SHDG + TT at the above doses (n = 121), all for 8 weeks. Before and after medication, we recorded the IIEF-6, erection hardness scores (EHS), traditional Chinese medicine syndromes (TCMS) scores, penile cavernous blood flow parameters and adverse reactions, and compared them between the 3 groups of patients.@*RESULTS@#After 8 weeks of treatment, all the patients showed significantly increased IIEF-6, EHS and TCMS scores in comparison with the baseline (P < 0.05). The total effectiveness rates in the SHDG, TT and SHDG + TT groups were 60.8%, 67.3% and 69.4% respectively based on the IIEF-6 scores, remarkably higher in the TT and SHDG + TT groups than in the SHDG group (P < 0.05), and 40.5%, 32.7% and 63.6% respectively according to the TCMS scores, markedly higher in the SHDG and SHDG + TT groups than in the TT group (P < 0.05). Single-center data manifested significantly increased peak systolic velocity (PSV) of the penile artery in the SHDG + TT and TT groups (P < 0.05). The improvement values of relevant parameters were remarkably higher in the SHDG + TT group than in the TT and SHDG groups, so were IIEF-6 scores in the TT than in the SHDG group, and TCM syndromes in the SHDG than in the TT group. No medication-related adverse events were found in any of patients after treatment, except for some mild side effects including muscle soreness and gastrointestinal reactions in a few cases, all soon relieved, none with abnormalities in blood and urine routine tests or hepatic and renal function indicators.@*CONCLUSIONS@#Shanhaidan Granules combined with tadalafil can significantly improve the erectile function and reduce TCM syndromes in ED patients, and therefore can be applied effectively and safely in clinical practice./.

ABSTRACT

Anastomotic leak is one of inevitable postoperative complications of rectal cancer. With the improvement of surgical techniques, the enhancement of the cognization of rectal cancer, and the development of surgical instruments, surgical procedures of rectal cancer are getting more sophisticated. The anastomosis is performed lower and lower, however the incidence of anastomotic leak is not significantly decreased. In addition, different from intraperitoneal anastomotic leak, the low rectal anastomotic leak after low anterior resection has many special issues in the diagnosis and treatment in clinic. The incidence of peritonitis caused by low anastomotic leak is low, the onset time is late, and symptoms of peritonitis are mild. So most low anastomotic leak is treated conservatively, second surgical repair or resection of anastomotic site is rarely performed, and proximal intestinal diversion is commonly performed. In the prevention of low anastomotic leak, some techniques and precautions during the perioperative period and identification of high risk factors might play important roles. Combined our clinical experiences, we introduced the diagnosis, treatment, prevention and research progression of low anastomotic leak after anterior resection of low rectal cancer, we hope it would be helpful.

ABSTRACT

OBJECTIVE@#To explore clinical effects of ultrasound-guided minimal traverse-cross technique repair for acute closed Achilles tendon ruptures.@*METHODS@#From January 2015 to March 2017, 20 patients with acute closed Achilles tendon rupture were treated by minimal traverse-cross technique repair with ultrasound guided. Among them, including 13 males and 7 females, aged from 28 to 49 years old with an average of(31.3 ±4.5) years old. All patients were single side injury. Fifteen patients on the left side and 5 patients were on the right side. The time from injury to operation ranged from 1 to 5 days with an average of (2.5±0.7) days. Operative time, postoperative complications were observed, and AOFAS score before and after operation at 12 months were compared.@*RESULTS@#All patients were followed up for 12 to 27 months with an average of(15.2±4.9) months. Operative time ranged from 33 to 65 min with an average of(43.7±5.6) min. Incision of one patient were continued oozing and improved after changing dressings, other patients were healed at stage I. No sural nerve irritation symptoms and palindromic rapture of heel tendon occurred. AOFAS score was improved from 65.2±7.4 before operation to 97.7±4.7 after operation at 12 months (t=22.5, <0.01); 18 patients got excellent results and 2 good.@*CONCLUSIONS@#Ultrasound-guided minimal traverse-cross technique repair for acute closed Achilles tendon ruptures, which promise minimal incision, protect sural nerve, ensure quality of tendon anastomosis and fixation, and is a ideal method for repairing acute closed Achilles tendon ruptures.

ABSTRACT

To study the effect of prescription for removing blood stasis and resolving phlegm on lipid metabolism and hormone levels of patients with polycystic ovary syndrome. Totally 93 cases of patients with polycystic ovary syndrome treated in our hospital from March 2013 to July 2014 were randomly divided into control group and observation group. The observation group was treated by oral administration of Quyu Huatan decoction, while the control group was treated with metformin. The result indicated that the total effective rate of the observation group was significantly higher than that of the control group(χ²=632, P<0.05). After treatment, the score of syndrome for traditional Chinese medicine in the observation group was significantly lower than the control group(t=13.865,P<0.05). After treatment, the levels of TG, BMI, TC in the observation group were significantly lower than the control group(P<0.05), and the difference of HDL between the two groups were not significantly. After treatment, the levels of FSH, LH and T in the observation group were significantly lower than those in the control group(P<0.05). After treatment, FBG and INS in the two groups were significantly decreased(P<0.05); the FBG in the observation group was significantly lower than that in the control group(t=3.981, P<0.05), and the INS level in the observation group was significantly higher than that in the control group(t=6.472, P<0.05). In this study, we can find that the Quyu huatan decoction for removing blood stasis and resolving phlegm has a better clinical efficacy in the treatment of polycystic ovary syndrome. It not only ameliorates the secretion of variety sex hormone, but also regulates the body's metabolism of glucose and lipid, so it is worth clinical promotion and application.

ABSTRACT

Expressions of GABA and GABA(A)R-alpha1 in the brain of rats poisoned by Tetramine were analyzed to explore the intoxication mechanism. Methods Sixty rats were randomly divided into control, sham poisoned, high-dose poisoned (1.0 mg/kg tetramine) and low-dose poisoned (0.1 mg/kg) groups. The expressions of GABA and GABA(A)R-alpha1 in the brain of the poisoned rats were detected and analyzed by immunohistochemistry and imaging analyzer. Results The expressions of both GABA and GABA(A)R-alpha1 were diffusely seen in the brains of the control and shame poisoned rat groups with a moderate expression level, whereas the expressions of both GABA and GABA(A)R-alpha1 were decreased in the brains of the high-dose poisoned group. In the low-dose poisoned rat group, the expression of GABA initially decreased and reached its lowest level 6 hours after poisoning, and then started to show an increase and reached the level of control groups at day 3. The expressions level reached its peak at days 5-7 after poisoning and remained above the level of control groups till 10 days after poisoning. Similarly, the expression of GABA(A)R-alpha1 in the brains of the low-dose poisoned group initially decreased and reached its lowest level 6-12 hrs after poisoning, and then started to increase and reached the level of control groups at days 7-10 after poisoning, respectively. Conclusion The expression of both GABA and GABA(A)R-alpha1 decreased in the brains of the high-dose poisoned rat group and these changes of GABA and GABA(A)R-alpha1 expressions may be associated with underlying mechanism of tetramine poisoning.

ABSTRACT

OBJECTIVE@#To find whether Bcl-2 and Caspase-3 take part in the pathophysiological mechanism of tetramine toxification.@*METHODS@#Sixty Sprague-Dawley rats were divided into normal control group, the sham poisoned group, high dose poisoned group, low dose poisoned group. High dose poisoned group were administered 1.0 mg/kg weight body tetramine by mouth, however low dose poisoned group was administered tetramine 0.1 mg/kg weight body by mouth. The rats of the sham poisoned group were administered water, and rats of normal control group were given nothing. Bcl-2 and Caspase-3 were detected by immunohistochemistry staining and the results were assessed by image analysis system.@*RESULTS@#The expressions of Bcl-2 and Caspase-3 in all organs were similar, ie, Bcl-2 and Caspase-3 expressed obviously in all organs of high dose poisoned group; in all organs of low dose poisoned group, they were hardly detected at 30 min after administration, however, at 3 h after administration, they could be detected obviously; Bcl-2 got to peak at 6 h-3 d after administration and Caspase-3 got to peak at 24 h-3 d after administration.@*CONCLUSION@#Bcl-2 and Caspase-3 take part in the pathophysiological procedure of tetramine poisoned rats.

ABSTRACT

OBJECTIVE@#To investigate the changes of excitability of rats' gastrocnemius after rats were died from mechanic asphyxia, so as to make an objective evidence for estimating of the early postmortem interval.@*METHODS@#After rats were sacrificed by mechanic asphyxia, the reaction of rats' gastrocnemius to variable electric stimulation at different period after death were recorded by electricphysiological method.@*RESULTS@#Changes in the properties of excitable tissue to electric stimulate in rats' gastrocnemius after mechanic asphyxia seems to be well correlated with the early postmortem interval and appeared a linear relation in certain period.@*CONCLUSION@#The regular changes of excitability in muscle might be a useful means for estimating of early postmortem interval.
